Request Exclusion

THIS EXCLUSION REQUEST FORM MUST BE FULLY COMPLETED BEFORE THE FORM IS SUBMITTED, INCLUDING THE ACKNOWLEDGMENT AND SIGNATURE. YOUR EXCLUSION REQUEST WILL NOT BE VALID UNLESS YOU COMPLETE ALL REQUIREMENTS AND PRESS THE BLUE “SUBMIT” BUTTON.

Exclusion requests must be received no later than July 6, 2026.

As ordered by the District Court, each request for exclusion must state the name, address, email, and telephone number of the person or entity seeking exclusion; and must state that the person/entity requests to be “excluded from the Settlement Class in In re PG&E Corp. Sec. Litig., Case No. 5:18-cv-03509 (N.D. Cal.).” Those requesting exclusion are also directed to provide the information requested in the Question 11 of Notice, including, but not limited to:

(a) a listing of the number of PG&E Securities (by CUSIP number and full name) that the person or entity requesting exclusion held on each of the requested dates;

(b) a listing identifying the number of PG&E Securities (by CUSIP number and full name) traded, and trading dates and prices of each such purchase/acquisition and sale, during the requested time periods;

(c) whether you also filed (or had filed on your behalf) a Rescission or Damage Claim in the Chapter 11 Cases and, if so, the claim numbers; and

(d) the signature of the person or entity requesting exclusion or an authorized representative (with proof of authorization).
